  • Completed by a Physician, Physician’s Assistant, ARNP, or RN under a physician’s supervision within the last 6 months.
  • Physician’s statement confirming freedom from communicable diseases.
• AHCA Level 2 Background Screening
  • Completed fingerprint-based background screening via AHCA Level 2.
